McGillvray ejected after an engine fire on takeoff from Baden.  This was his second CF-104 ejection.  Aircraft landed in a cemetery.  Board of Enquiry determined that the progressive engine failure resulted in eleven major warning lights in 7 minutes and 42 seconds.

In storage at Scottish Aviation, Prestwick, UK when struck off.  Seen at Prestwick on 4 June 1972, waiting delivery to Norway.  Reported stored at Prestwick, Scotland on 2 December 1972.  To Norway on 9 October 1974, their serial number 797.  Arrived with 1765:50 airframe time, had 1871:10 when it crashed.  Crashed on 29 July 1975, at Revtind, between Beiarn and Blakkådalen. The pilot, Lt. Per Yngvar Aas, was killed.  He was on a routine navigation training mission and probably hit the mountainside.  The wreck was found 1150 meters above sea level.

Seen at Prestwick, Scotland in December 1972.  With No. 421 Squadron, 1 Canadian Air Group, at time of crash.  Fuel pressure sense line had been improperly torqued, worked loose, resulting leak ignited by bleed air when flaps down selected for landing.  Multiple explosions and fire resulted.  Pilot Capt. R. Harrison ejected, received minor injuries.  Aircraft landed near the village of Muchenschop, West Germany.
